SERVICE PROVIDER FEEDBACK What do you think about the Services you receive? Was the Provider okay? Did they listen to what you had to say? How were ACC/MoH/Work and Income in the process? For many years people have been telling us about the experiences that they've had when they've worked with various Service Providers for various reasons. This could be for attendant care assessments, help getting back to work, housing modifications, vehicle and technology assessment services etc etc. Also the funders (ACC/Ministry of Health/Work and Income) are often asking for feedback about how consumers feel about the services that they've been referred to. Some of these experiences have been very positive and some have not, so we'd like you to please share your experience with us so that feedback (both positive and negative) can get back to the funders about what ACTUALLY is going on. There are 17 questions which should only take a few minutes so please feel free to give as much or as little detail as you like. When you click 'Add' on the first screen a second screen will show your answers and you can make changes by clicking 'Cancel'. If it's all okay then click 'Confirm' on this second screen and the information will be saved on our database. This feedback can be completely ANONYMOUS.
