Accessing the Service
The service is available to residents within the Canterbury DHB area and
patients of the Burwood Spinal Unit. Referrals from outside these areas may
be considered on a case by case basis. The main disability groups serviced
are:
- Spinal Cord Injury
- Traumatic Brain Injury
- Neurological Dysfunction
- Upper Limb Amputation
- OOS
- Birth Trauma
- Deafness
- Vision Impairment
Referrals should be made through an ACC Case Manager, a Needs Assessment
Service, Workbridge, an occupational therapist, a speech-language therapist
or a doctor. Clients may self-refer in some circumstances.

Making a Referral
Referrals to the Burwood Assistive Technology Centre should be made on the
ATC Referral Form. (.pdf)

The Referral Form should be completed by an ACC Case Manager, a Workbridge
Case Manager, a Needs Assessment Service Coordinator, an occupational
therapist, a speech-language therapist or a doctor.
Where the client being referred is an ACC Claimant, the ACC Case Manager MUST
approve the referral and ensuing assessment.
All sections of the Referral Form should be completed, with particular
attention being given to the "Goals to be Attained Through Use of Technology".
Please remember that a goal is something the client is trying to achieve,
not a piece of equipment (e.g. "effective communication in writing" rather
than "a speech recognition computer").
In order to complete the assessment it is usually necessary for us to access
recent relevant assessment and provider reports. It is helpful for these to
be sent at the time of referral to avoid delays later.
